The topics have been carefully chosen to cover common differential diagnoses to common symptoms. 45. Medicine facts (side effects, onset of side effects, & off-label therapeutic uses) Common facts for common meds commonly used:
�
� QT interval prolongation: can be caused by a lot of meds like quinolones, antiarrhythmic meds (amiodarone/tykosin/procainamide), psychiatry meds (fluoxetine/quetiapine/haldol), pain meds (methadone), & a lot of others. This can evolve to torsades de pointes (TdP), which is a unique V tachycardia. Monitor QTc before & on a regular basis after starting & consider stopping the meds if QTc >500-550. Check K & Mg & replete as needed. Treat TdP w/ ↑ dose IV Mg along w/ ACLS.
� �Statins: ↑ Creatine kinase (from muscles) & AST/ALT. Statins can help maintaining sinus rhythm after a fib cardioversion (ACE inhibitors can help as well maintaining sinus rhythm), but it is still NOT used for that purpose alone.
� �Pregnancy & antiemetics: non-pharmacological methods are preferred like eating small meals, avoiding strong odors, getting enough sleep, & avoiding fatigue. Evidence is insufficient regarding the safety of most of the meds. Many class C antiemetic meds are prescribed during pregnancy when benefits> risks. Meds: multivitamins, Zofran, Phenergan, Reglan, & Meclizine.
� Cirrhosis & Tylenol: up to 2 g/day is safe (up to 4g/day in healthy person). Toxicity is when levels are up to 10-15 g/day. Acetylcysteine is the antidote.
� Cirrhosis & increased Ammonia: Use Lactulose (laxatives, monitor) & Rifaximine (abx). NO need to monitor Ammonia levels & follow up clinical response (up titrate lactulose dose to target 3-4 bowel movements)
� Pulmonary HTN & Remodulin (peripheral prostacyclin vasodilator) infusion pump: do NOT hold because rebound pulm HTN may occur & could be catastrophic.
� Metoprolol & Coreg for A fibTx: both can rate control the heart. Coreg has alpha-1 blocking activity & can cause HoTN (metoprolol causes less HoTN). So choose coreg if you want to control HTN along w/ slowing the rate.�

Highly recommend!

I was looking for one thing to read before I start residency but a lot of the materials out there seem like it will take too long to read and may be counterproductive because it gets too detailed--not this book. This book is the perfect balance of information presented in an easily absorbed format. It's like having a refresher on the last 4 years of medical school AND getting a sneak peek of what I am going to see during 3 years of residency. Exactly what I wanted and needed. Highly recommend.

Finally, a straight forward, concise and user friendly guide for residents. As a recently graduated IM resident, I wish I had this book in my intern year to help guide me. Obviously, it is not meant to be a comprehensive resource, but when you are on the hustle and bustle of the wards (especially before morning report/rounds) this guide really helps with formulating a plan. I stress "helps" because it shouldn't be used as a sole source for decision making, but is a very nice and easy quick guide to be kept in your lab coat. Has all the bread and butter medicine case and then more. Would definitely recommend all med students on IM/FM rotations and IM interns (and even residents) to maintain a copy of this book in their lab coat at all times!! Great job!

An excellent book to read in the intern year. It covered at least 90% of the topics and diseases that mostly you will see during residency. And each topic was written in a way easy to read and digest . you will get an idea about hospital way in managing patients, not the way we learned during study in order to answer exam questions.

It's All Right Here. All of it.

I shouldn't even be writing this review because doing so only creates my competition. I'm writing it for Robert McKee out of respect and love for him as one of the best instructors I've never met. I have both STORY and now, DIALOGUE in hardcover, on my Kindle, and the audio version (he narrates his books) in my headphones. I read it, I listen to it and take it with me when I travel. Sometimes I listen and read at the same time. Most nights I listen to the audio version in my sleep. In other words, I eat, sleep and breathe this book. I did the same thing with Story, his other book. I may go to my grave never fully comprehending the vast wealth of knowledge contained in these two books. That's OK. What I can tell you is this. With nothing but STORY as my guide, my very first screenplay took seven months and thirty-five drafts from start to finish. But. That screenplay became a Hallmark Movies and Mysteries feature film. The producers liked it so much they gave me another assignment. That one took six weeks, and they bought the first draft. I'm now working on my fifth script; this one is in the six-figure category, with five figures upfront just for the Treatment. And I owe it all to everything I've learned from studying Robert McKee, supplemented by what I learned from everyone else. Over the past thirty years, I’ve studied with forty plus instructors and highlighted hundreds of books and listened to dozens of recorded seminars. All that information is summed up and thoroughly explained in Robert McKee's two books STORY and now DIALOGUE. I won't live long enough to absorb everything he teaches. And I still study two hours a day as a warm up for my writing. I’d recommend Aaron Sorkin, Warner Hertzog, William Goldman (both Sorkin and McKee say he's the greatest), Blake Synder, Chris Vogler, Michael Hauge, William Akers, and anybody else you can find who’s willing to share their knowledge. Because you never know when a concept you didn’t realize you didn’t understand or needed is going to show up. Especially when presented from a different perspective. Having said that, if you are serious, and I mean dead serious, about becoming a working screenwriter, or any other kind of fiction writer for that matter, then you have no choice but to study McKee like your literary life depends on it. Buy the hardcover, buy the Kindle version, and buy the audio version of DIALOGUE and STORY. And supplement these two works with any other material that speaks to you. If you do this, you will become a first class screenwriter or novelist or playwright, because all three genres are only different ways of presenting a Story. If you can’t commit to this, unless you’re a genius or prodigy, you’re wasting valuable time which could be spent following your true life calling. But if your heart’s desire is to become a working writer, then sooner or later you’ll have to know everything in McKee’s two books. So, you might as well bite the bullet and jump in head first. It's all right here in STORY and DIALOGUE. All of it. Thank you, Mr. McKee. You, sir, changed my life.-- Jimmy Hager

ONE OF THE TWO BEST BOOKS ON SCREENWRITING

Probably the best book on screenwriting ever (besides Egri), though there is also much here for the novelist and playwright. I am a professional TV writer, of long-standing (35 years), and I can tell you I used this book to figure out how to fix the problems of a complex pilot I'm writing; the author truly " guided me home." And lest you think I'm a McKee sycophant, I am not. I found little in STORY for me. The only thing I disagree with in DIALOGUE is that the author sells his own work short: it isn't just for those who are "lost" in their writing, like me, and the student, it's for anyone who writes fiction for a living, in any form, no matter how much experience they have. It's that good.

Excellent supplement to 'Story' - with examples of speech from popular stories that really drive home the content. It shows how to immerse dialogue into your scene(s) to give the words more meaning, and in a tension-raising-cycle, turn and give the scene itself more impact. I enjoy writing dialogue more with this better perspective of what to look for, and what questions to ask before giving my characters words. This is a book that will get multiple re-reads for reference and increase in understanding as I look to improve my craft and skill-set. Thanks Robert and company!

The call of the streets....the desire to do the right thing...the need for love...

The "Trophy Wife" is another banger from Ashley and Jaquavis. Kalil is a young man fresh out of jail and he is on a mission to do the right thing. The right thing means taking care of his little girl Jada and working an honest 9-5 job for some shady Italians. Kalil is used to hustling and balling out of control with unlimited cash, but to stay out of jail for his daughter he will do the right thing...for a little while at least.

When Kalil meets the lovely London, Jada's dance teacher, he is immediately drawn to this beautiful woman. Kalil soon learns that London's husband is abusing her and she has the bruises to prove it. Kalil and London fall in love, and London knows she is playing with fire. London wants to feel real love and not like someone's property or punching bag. London and Kalil begin an affair and this sets off a chain of events that will change there lives forever!!!

This book will leave you crying and asking why. All Kalil wants to do is provide for his daughter and baby's mother. But after he came home from doing four years in jail, he finds his baby's mother cheating on him with his rival. After that he tries to get back on his feet. He doesnt want to get back in the drug game, but working a 9 to 5 isnt his thing. When he finds out that his rival has been touching his eight year old daughter, the old him comes back. Then when he took his child away from her mom he has to start taking care of her. While he is doing that he meets London, his daughter's dance teacher. She was being abused at home, and when he tries to take her away from that he finds out that the person who is abusing her is the same person that helped him get to the top. Then when the guy finds out what is going on, all hell breaks lose.

I am now a fan of Ashley and Jaquavis. However, this book was a let down. It had a great start with Kalil telling his story from jail. But somehow it justed flopped after a while. The chapters started to become too predictable and seemed kind of amateurish. Kahil started to get on my nerve simply because he couldn't take no for an answer and in the end lost everything. I had to force myself to just finish to the end. It is not a great read and i just gave away the book.

About the Author

Peter Wallensteen is a pioneer in peace research, having worked in the field since the 1960s, and Since 2006 is Richard G. Starmann Sr. Research Professor of Peace Studies at the Kroc Institute at the University of Notre Dame. He was a founder of the Department of Peace and Conflict Research at Uppsala University and the first holder of the Dag Hammarskj�ld chair in peace research in Sweden in 1985. He directed the Uppsala Conflict Data Program, which has become a leading provider of data on armed conflict and other forms of organized violence around the world.
